Meet Dr. Thomas Evans

Physician, Coach and Advocate for  Well-being in Healthcare.

I’ve spent my career working in emergency medicine—helping people through some of the most critical, complex moments of their lives. Over the years, I’ve also witnessed the toll this work can take: the burnout, the disconnection, the quiet sense of being overwhelmed. I’ve felt it myself. That’s what led me to expand my work beyond the ER and into the world of coaching.

Today, as a certified personal, executive, and health & wellness coach, I partner with healthcare professionals and leaders who are ready to reconnect with their values, clarify their purpose, and create space for real well-being. My approach is collaborative, compassionate, and grounded in the belief that you already have the wisdom within you—I’m here to help you access it.

Background & Purpose

A Brief Introduction.

For nearly two decades, I’ve dedicated my career to emergency medicine—serving patients, mentoring medical students and residents, and leading within my hospital community. I trained as an emergency physician at the University of Massachusetts Medical Center, where I also served as Chief Resident. Since 2009, I’ve been an attending emergency physician at Milford Regional Medical Center in Massachusetts, where I now hold several leadership roles, including Co-Assistant Medical Director of the Emergency Department and Assistant Chair of Emergency Medicine.

Throughout my career, I’ve worn many hats: clinician, educator, administrator, and mentor. I serve as Co-Site Director for the UMass Emergency Medicine Residency, oversee medical student education through the UMass Emergency Clinical Problem Solving course, and sit on both the Medical Executive and Flow Committees. My passion for guiding and supporting others has always extended beyond the bedside.

But like so many in healthcare, I’ve also felt the intense weight of burnout. That personal experience led me to expand my impact in a new direction.

I pursued advanced training in coaching—earning certifications in Positive Psychology Health & Wellness Coaching, Team Coaching, and Executive Coaching through the College of Executive Coaching, an International Coaching Federation Accredited Coach Training Program. I’m proud to be an Advanced Certified Personal and Executive Coach and a National Board Certified Health and Wellness Coach.

Today, I work with fellow physicians and healthcare leaders who are searching for something more—more clarity, more balance, more meaning. My coaching philosophy is grounded in the belief that we all hold the keys to our own fulfillment—we just need space, guidance, and support to reconnect with what truly matters.

Outside of my clinical and coaching work, I continue to stay involved in community service and governance as a Corporator and Foundation Board Member of Milford Regional Medical Center, and through my volunteer work with Project Just Because in Hopkinton, MA.

Whether I’m in the ER, in a coaching session, or mentoring the next generation of healthcare providers, my mission is the same: to foster resilience, purpose, and well-being in those who give so much of themselves to care for others.

In my final year of residency, I wasn’t sure about what I was going to do after graduating. Did I want to do fellowship? Go into practice? Relocate? I felt stuck and indecisive.

That’s when I found that Tom was offering coaching. I wasn’t sure what to expect but from our first meeting, I could tell that Tom was truly invested in helping me succeed. He had prepared various exercises that helped me understand my values and priorities, which we worked through over the course of a few months. He kept me accountable with various action items, which prevented me from procrastinating. Tom helped me realize that I needed to be closer to family, and looking back, it was definitely the right decision.

Since then, I have worked with him on everything — from finances to building community, interpersonal relationships and sleep habits.  As I have gotten to know him, I can see that he cares deeply about helping others live their lives authentically, with purpose and intention. An accomplished EM physician, he has a wealth of experience in healthcare that he draws upon. But, what stands out is his compassion, patience, and humility. I never feel judged when I work with him, and he makes it clear that his clients are a top priority.

Simply, working with Tom has been transformative over the past several years and I would definitely encourage any healthcare professional to work with him!

Working with Tom was truly life-changing for me. As a busy resident physician, I struggled for years to stay consistent with my fitness goals. Over the course of a year, Tom helped me get back into the gym and stay there. During that time I lost 12 pounds, took my body fat % down 5 points and even hit a personal record in a Spartan Race — all while balancing my demanding schedule in the hospital. Along with this came new friends at the gym and a new sense of purpose outside of the hospital.

What stood out the most was how he broke down my big goals into smaller, manageable steps and was always patient and supportive when I hit roadblocks. For example, when I struggled to find time to work out, he helped me adjust my sleep routine so I could train early in the morning and miss fewer sessions (I was always a night owl before).

He was flexible and met with me however worked best — in person, over FaceTime, or kayaking on a lake. I can’t recommend him highly enough — 10/10!

Working in emergency medicine is deeply rewarding, but it’s no secret that it can also be exhausting, overwhelming, and at times, disheartening. On top of the daily grind of working shifts with sicker, more complex patients, there are administrative burdens and systemic frustrations that can take a toll on morale and overall well being. Like many of my colleagues, I found myself burning out — losing sight of the purpose and passion that brought me into medicine in the first place.

Fortunately, one of my closest friends is also a fellow emergency physician who just happens to be one of the most humble, kind and thoughtful people I’ve ever known. Tom Evans is not only an exceptional emergency physician but also a truly gifted coach. Through his professional coaching, he helped me refocus and rediscover the joy in what I do. His insight, empathy, and practical guidance reminded me that caring for others doesn’t have to come at the cost of caring for myself.

Dr. Evans understands this field from the inside — its challenges, its pressures, and its unique rewards. His support has made a real difference in how I show up for my patients and for myself.
